Research Intention
研  究  的  初  衷

在台大牙醫的學習路程中,
我選擇了齒顎矯正學作為受訓項目。
它融合了 『職人匠心的手工藝美學』,
以及 『極富彈性和變化的治療方式』。

跟診學習的時候,我發現一件有趣的現象:
矯正後牙齒變整齊,有些病人卻覺得治療後變得不好看了?!


這個狀況讓我不禁去思索,
是否我們矯正醫師所追求的目標,
和病人想要達到的美觀有落差呢?

我們能否透過有計劃的治療設計,
讓病人在治療後都能有更美麗的笑容呢?

基於以上原因,我選擇進入北醫,繼續進修齒顎矯正研究所。

Objectives

The purpose of this study was to investigate the differences in smiling and soft tissue changes from resting to smiling in layperson with different craniofacial features in frontal, oblique, and lateral views photographs.

Additionally, the study aimed to explore whether smile characteristics at different views were related to gender, facial morphology (ANB, FMA), and occlusion (OJ, OB). By understanding the effects of these factors, the study aimed to predict the changes in smile characteristics of patients with different craniofacial features, and serve as a reference for communication with patients before orthodontic treatment, to achieve goal of transparent treatment information.

Methods

200 Participants in this retrospective study were recruited from pre-orthodontic treatment patients in Taipei Medical University Hospital between 2019 and 2021. Patients above 10 years old were included and their extra-oral photographs and lateral cephalograms were collected. The cephalograms were analyzed to classify patients according to their facial types (ANB, FMA) and occlusal relationships (OJ, OB), and all patients' pre-treatment smiling photographs were measured for landmark positioning and linear measurement of smile analysis variables. 

 

Independent sample t-test was used to compare smile characteristics between males and females, while one way ANOVA analysis was used to compare whether there were differences in smile characteristics between groups with different facial types (ANB, FMA) or occlusal relationships (OJ, OB), with Scheffe post-hoc tests being performed to identify which specific groups showed significant differences. 

 

Furthermore, we measured the smile characteristics variables from relaxing to smiling positions and compared whether there were differences in soft tissue changes during smiling among different groups. 

 

Finally, multiple linear regression analysis was performed to investigate the potential factors related to various craniofaciodental types that may influence smile characteristics.

Conclusion

Based on the findings of smile characteristics analysis, we orthodontics should put more emphasis on oblique view photograph when evaluating the pre-treatment data, not just frontal and lateral view, which can give more information about the patients' smile esthetics. Also, more smile characteristics differences were observed in the lower third of the face rather than middle third, where should be focused more during smile analysis. 

 

Furthermore, from the findings of smile changes analysis, FMA angle showed no influences on any smile variables, so skeletal vertical dimension may be the least important factor to be considered. At last, the lower lip is the part that showed most differences between the groups, so we should pay more attention on the lower lip when analyzing the smile changes afterwards.
